Students may begin registering for the 2025 fall season. To register, a student-athlete must return a completed PIAA Pre-Participation Physical Form. These forms are available under the ATH DEPT INFO menu. Click on PIAA Registration Info, then click Download PDF at the bottom of the page. These forms are also available in the high school, middle school, and athletic offices.

There are five pages to the form. The first four pages must be filled out by the student-athlete and the parent/guardian. Page five must be filled out by a physician, physician’s assistant, or nurse practitioner (Authorized Medical Provider).

According to PIAA rules, students must receive a physical exam from an Authorized Medical Provider on or after May 1 each year. If a student-athlete plans to attend the school-sponsored physical, a completed Pre-Participation Physical Evaluation form signed by the parent/guardian and the student must be returned no later than Monday, May 19. PIAA Pre-Participation Physical Forms can be returned to the high school or middle school offices or via USPS mail to the Athletic Office at 929 Lakeshore Drive, Leesport, PA 19533. By having all forms by May 19, our Athletic Trainers and Team Physicians can review a student's health history prior to the physical exam, which helps the process move quickly and smoothly.

On Tuesday, May 20, Schuylkill Valley School District will hold fall sports physicals in the high school gymnasium area. Physicals will begin at 6:00 pm. When arriving for physicals, please enter the high school gymnasium lobby and check in at the registration table. There is no cost for these athletic physicals.

High school football begins mandatory heat acclimation on Monday, August 4. The golf season also begins on August 4. To start the season on time, PIAA Pre-Participation Physical Forms must be turned in no later than Friday, August 1.

All other high school sports, including cross-country, cheerleading, field hockey, boys’ and girls’ soccer, girls’ tennis, and girls’ volleyball, begin the season on Monday, August 11. To start the season on time, PIAA Pre-Participation Physical Forms must be turned in no later than Friday, August 8.

The junior high sports season, including cross-country, cheerleading, football, field hockey, boys’ and girls’ soccer, and girls’ volleyball, begins on Monday, August 18. To start the season on time, PIAA Pre-Participation Physical Forms must be turned in no later than Friday, August 15.
