SIGN-UPS HAVE BEGUN FOR THE FALL SEASON!

Students may begin registering for the 2025 fall season. To register, a student-athlete must return a completed PIAA Pre-Participation Physical Form. These forms are available under the ATH DEPT INFO menu. Click on PIAA Registration Info, then click Download PDF at the bottom of the page. These forms are also available in the high school, middle school, and athletic offices.There are five pages to the form. The first four pages must be filled out by the student-athlete and the parent/guardian. Page five must be filled out by a physician, physician’s assistant, or nurse practitioner (Authorized Medical Provider).According to PIAA rules, students must receive a physical exam from an Authorized Medical Provider on or after May 1 each year. If a student-athlete plans to attend the school-sponsored physical, a completed Pre-Participation Physical Evaluation form signed by the parent/guardian and the student must be returned no later than Monday, May 19.
